This report should be submitted three times a year for each student; at the beginning enrollment of each year, by 3 Jan of each school year and at the end of your school year or 3 July whichever comes first.

180 days required for the school year which starts 4 July and ends 3 July. Report cumulative (for example as
3 of 180 or 98/180) for each report.
Each Student allowed 20 Teacher Days (for illnesses or teacher preparation) of the 180 required. Please annotate anything out of the normal during the reporting period.
A day of attendance (at a minimum) is 3 to 4 core subjects being taught. Or at least 3 to 4 hours of instruction which may include field trips.
Grade Level & General Curriculum
Such as 4th Grade & Abeka, Bob Jones, Unit Study or others.
